Just wanted to update this thread:
As it turns out, Chris was able to do this after discussion with our account team and professional services.
A qcow2 image was provided, uploaded by the OpenStack admin, and made available through the OpenStack Catalog.
The disk requirements are provided here https://cd.delphix.com/docs/latest/deployment-for-kvm.

I don't believe OpenStack is a supported platform at this time. I've checked with our documentation team and done some searches myself, too.
